故事发生在明朝中叶。锦衣卫杨傲（彭敬慈 饰）与御医李鹤龄（黄志忠 饰）本是同门师兄弟，因杨傲爱上师兄鹤龄未婚妻霓裳（吕一 饰），便嫁祸师兄奸杀慧妃，为救李鹤龄，霓裳被迫嫁给杨傲。李鹤龄无奈远走他乡，逃至山西原仓县开了家医馆谋生。多年后，因一桩离奇案件，他与原仓县令刘福星（张卫健 饰）相识相交，联手破获许多疑案。在一桩杀人案中，查获杨傲手下范泰（房振华 饰）的情妇，从其口中得知当年杨傲陷害自已的真相。鹤龄上京求皇帝（宗峰岩 饰）为自己平反，得到了旧爱霓裳的鼎力相助，杨傲一怒之下杀死霓裳。鹤龄联手福星除掉杨傲这颗灾星，谢绝御医首席的美差，和福星重返江湖......

In the early 1970s, the Governor of Hong Kong decided to clean up the police force. The ICAC was established, whose operation branch was headed by Yim. Bong and Unicorn also joined the ICAC.
